Box 6015 Indianapolis, IN 46206-6015 Dear Ms. Fliss: As per Supplement to Master Agreement For hazardous Substance Or Petroleum Response Services between the Indiana Department of Environmental Management and Earth Tech, please find the Monthly Report of Operations of the Illinois Central Spring Treatment Plant, Bloomington Indiana for the reporting period of November 2001. Operational Status - An extended period of wet weather occurred during reporting period between November 24 and November 30, 2001. Daily records are summarized on the attached reports. The maximum rainfall during a 24-hour period was 1.13 inches with a total of 3.64 inches recorded for the month. A total of 3,228,032 gallons of water was diverted to storage with 711,403 gallons returned for processing and 1,252,099 gallons of water passively treated through the SRS Tanks as an overflow during these storm events. At the end of the month 1,264,530 gallons remained in storage. No bypass of treatment occurred during this storm event. Corrective Measure Taken or Planned - None to report for November 2001. Analytical Results - Analytical results for the reporting period are attached to this report on the Event Sampling Analysis Log.
Unexpected Maintenance Requirements - There were no unexpected maintenance requirements reported during this period. Evidence Of Trespassing - There was no evidence of trespassing at the facility during the reporting period. NOTE: This reporting is filed electronically with the Project Manager of the State of Indiana, Department of Environmental Management (IDEM). The original reporting including signatory requirements are maintained at the ICS Water Treatment Facility at 1550 West Third St., Bloomington, Indiana 47403. Copies of this reporting are available for review upon written authorization of the IDEM Project Manager. The original documents on file at the ICS Water Treatment Facility shall prevail over any other such outstanding copy or reproduction. Discrepancies, changes, or modifications to copies should be confirmed with the original documentation. Report from Visitors Log - The Visitor's Log is available for inspection at the ICS office. Copies of this log are available to authorized personnel upon request and authorization of the IDEM Project Manager. There were no telephone calls or requests for tours received from the public by Earth Tech. Summary of Costs - Total costs for this Project for the reporting period of November 2001 (through 11/23/01) was $22,326.97. The summary of cumulative project costs and comparisons to the project budget are attached to this report. This cover letter and attachments complete the monthly report for Illinois Central Spring.
